TERMS OF SERVICE

These Terms of Service apply to purchases and program enrollment through Elite Anti-Aging Center. Please review the terms shown for a particular service before you pay. Your purchase also remains subject to our Subscription and Cancellation Policy, Shipping Policy, and Privacy Policy, as applicable.
Consultation and eligibility
The initial testosterone replacement therapy consultation costs $99. Lab testing is charged separately from the consultation and monthly program fee. The doctor will complete the consultation and review your lab results before deciding whether the program is appropriate for you. Paying for a consultation does not guarantee program eligibility, a prescription, or a particular treatment outcome. The consultation fee is separate from the monthly program price. Charges for a completed consultation are nonrefundable, except as required by law or to correct a billing error.
Program enrollment and payment
If the doctor determines that you qualify and you choose to enroll after the consultation and lab review, the testosterone replacement therapy program costs $99 per month. This charge is for program participation. Your initial 12-month term begins when you enroll and your first $99 monthly program charge is made. Before enrollment, we will show the first billing date, what the program includes, and any additional charges. You authorize us to charge your selected payment method monthly under the terms you accept at checkout.
After the initial 12-month term, billing continues month to month at $99 per month until cancellation takes effect under our Subscription and Cancellation Policy. We will disclose any proposed price change in advance and honor rights provided by applicable law.
Clinical decisions
The treating clinician decides whether treatment, medication, dosage, testing, and continued participation are appropriate. A subscription does not guarantee that a prescription will be issued or renewed. Please contact the clinical team if your health changes or you have a question about treatment. This website and its checkout process are not for emergencies. Seek immediate medical care for an emergency.
Prescription fulfillment
All medications prescribed through the program are dispensed directly by our compounding pharmacy, subject to the pharmacy's review and applicable law. A prescription must be valid before the pharmacy can dispense it. If a prescribed medication is compounded, that preparation is not FDA approved. Instructions provided by your clinician and the dispensing pharmacy govern its use and storage. Our Shipping Policy explains how prescription orders are handled.
Cancellation, returns, and refunds
To cancel the monthly program, send written notice to info@eliteantiagingcenter.com or use the cancellation option in your online account, if available. The initial 12-month commitment and 60-day notice requirement are explained in the Subscription and Cancellation Policy. The initial term may end early if your treating clinician determines that continuing treatment is medically inappropriate or you no longer qualify. A clinical decision to stop treatment does not require 60 days' notice.
Dispensed prescription medication cannot be returned. Charges for medication already dispensed and services already provided are nonrefundable, except as required by law or to correct a billing error. If care cannot be provided after a clinician determines that treatment must stop, future charges and any applicable prepaid amounts will be handled as described in the Subscription and Cancellation Policy and under applicable law.
Your information
Provide accurate contact, shipping, and payment information and keep it current. Use the patient portal or contact the clinical team for medical questions; do not include sensitive medical details in ordinary email. Our Privacy Policy explains how information submitted through the website is handled. Medical information may also be subject to separate patient privacy notices provided by the clinical practice or pharmacy.
Changes and contact
We may update these terms for future purchases or enrollment. Changes to an existing paid subscription will be communicated as required by law and will not remove rights you already have. For questions about these terms, billing, or your subscription, email info@eliteantiagingcenter.com.
