SUBSCRIPTION AND CANCELLATION POLICY

Program term and billing
The initial consultation costs $99. Before you enroll, the doctor will complete the consultation, review your lab results, and determine whether the testosterone replacement therapy program is appropriate for you. If you qualify and choose to enroll after that review, the program costs $99 per month. The consultation fee is separate from the monthly program charge. Your initial 12-month program term begins when you enroll after the consultation and lab review and your first $99 monthly program charge is made. The first billing date and services included in your plan are shown before you enroll and in your order confirmation. By enrolling, you authorize Elite Anti-Aging Center to charge your selected payment method $99 each month during the initial term.
After the initial 12-month term, the program continues on a month-to-month basis at $99 per month until cancellation takes effect under this policy. Any price change will be disclosed in advance, and your rights under applicable law will be honored.
Cancellation and written notice
You may give written notice of cancellation at any time by emailing info@eliteantiagingcenter.com or submitting a cancellation request through your online account, if available. We will confirm receipt in writing and provide your cancellation effective date. Please do not include sensitive medical details in an ordinary email; contact the clinical team through your patient portal or office for medical questions.
Except for the medical circumstances described below, the initial 12-month commitment remains in effect. To end the program at the close of that term, we must receive your written cancellation notice at least 60 calendar days before your 12-month term ends. If we receive notice later, cancellation takes effect 60 calendar days after we receive it, but no earlier than the end of the initial term. After the initial term, cancellation takes effect 60 calendar days after we receive written notice. Monthly charges scheduled before the effective date remain due, except where applicable law requires otherwise. We will not charge for a billing period beginning after the effective date.
Medical exception
The initial 12-month commitment may end early if your treating clinician determines that continuing the program is medically inappropriate or that you no longer qualify for treatment. Contact the clinical team promptly if you have a medical concern. A clinical decision to stop treatment is made by the treating clinician and is not conditioned on completing the subscription term or giving 60 days' notice. Once we receive and confirm that determination, we will stop future program charges for care that will not be provided and tell you the effective date in writing. Any amount paid in advance for services or medication that cannot be provided will be handled in accordance with applicable law. A request to stop treatment for other reasons does not by itself cancel the initial payment commitment, subject to applicable law.
Returns and refunds
Dispensed prescription medication cannot be returned. Charges for the completed consultation, medication already dispensed, and services already provided are nonrefundable, except where required by law or to correct a billing error. If an order is damaged, incorrect, or has not been received, contact info@eliteantiagingcenter.com promptly so we can review it. Do not mail medication back without instructions from the dispensing pharmacy.
Clinical care
Enrollment does not guarantee a prescription or continued treatment. Prescriptions, dosage, monitoring, and decisions to continue or discontinue treatment are made by a licensed clinician based on medical judgment and applicable law. If you need to stop or change treatment, contact your clinician; do not wait for a subscription cancellation date.
